2) Diagnostic: Through our care planning process our Case Manager develops a plan of care that addresses the clients physical, physiological and custodial needs.
3) Design: In conjunction with the client, family and other care providers. We set plan in place to ensure your care needs are met. Our coordinator works with the Case Manager to hand pick the best caregiver(s) for you.
